• Food Establishment Residential Kitchen - Application permit for a residential kitchen initial review. 
• Food Establishment Plan Review - Application for a plan review. Plan reviews must be filled out and submitted to the Health and Human Services Department when building new or renovating a current space. 
• All Food Permit Types: Annual - Food establishment permit (Includes: mobile food trucks and ice cream trucks) 
• Temporary Food Provider - Temporary food permit (DUE 30 days BEFORE the event date)

Pool

• Pool Plan Review - Pool plan review application (opening a pool guidelines)
• Pool - Permit application to operate a swimming pool

Miscellaneous

• Keeping of Animals - Keeping of animals application permit. 
• Hotel/Motel - Hotel/Motel application permit. Expires yearly on March 31st
• Recreational Camp - Recreational camp permit. DUE 30 days BEFORE the start of the camp.
• Tanning Establishment - Tanning establishment permit
• Trash Hauler - Removal or transport of garbage, offal, and other offensive substance service application. 
• Selling Tobacco Products - Application permit to sell tobacco products. 
• Farmer's Market - Farmer's Market permit application. 
• Worker's Compensation - Worker's Compensation Insurance Affidavit: General Businesses
